UCSF Cancer, Fertility, and Fertility Preservation Study
Drs. James Smith and Mitchell Rosen of the Departments of Urology, Obstetrics, Gynecology, and Reproductive Sciences at the University of California, San Francisco are conducting a study titled the "UCSF Cancer, Fertility, and Fertility Preservation Study," to determine the impact of a cancer diagnosis and cancer treatment on men and women’s ability to have children.
With your help, we hope to provide cancer and fertility specialists more insight into the ability of men and women to protect their fertility prior to cancer treatment. With your help, we hope to better understand the experiences of patients diagnosed and treated with cancer so that we can significantly improve the ability of cancer survivors to start families in the future.
If you decide to participate, you will take an ANONYMOUS online survey that will take less than 20 minutes to complete. No personally identifiable information will be asked of you. All answers you provide will be completely anonymous. Again, we greatly appreciate you taking the time to help by sharing your experiences with us.
Thank you for considering participation in this study.
